Before You Book Online...
Here's some important information to consider.
If you are a private patient (including those with private health cover) and want to book at our Vic Park (Leisurelife) or Kelmscott clinic, you don't need a referral and you can skip right ahead to our online bookings page. Just remember to book an appointment listing under "Private Patients & Care Plans (Medicare)".
Care plan referrals:
If you have been referred by your GP under a care plan, go ahead and book online at our Vic Park or Kelmscott clinic. You'll need to bring in your care plan and referral letter, unless we already have them on file. Medicare will cover $55.10 per appointment, so you'll need to cover the rest (pension & concension card holders get a 15% discount). If you plan to claim your rebate in clinic, you'll need a physical debit card (not a credit card). Remember to book an appointment listing under "Private Patients & Care Plans (Medicare)".
If you have a valid referral from your GP (form D0904), go ahead and book online at our Vic Park or Kelmscott clinic. You'll need to bring in your referral, unless we already have it on file. DVA will fully fund your appointment. Remember to book an appointment listing under "DVA Claimants".
On our bookings page, you'll see an option for "Mobile clinic". This is available only for valid workers compensation, motor vehicle accident, and income protection claimants, as well as NDIS participants. Due to the logistical nature of booking mobile services, you'll have to contact us directly to book an appointment. Nonetheless, feel free to peruse our practitioners schedules and availability. We do not yet offer mobile services for private, care plan and DVA bookings.
Workers compensation / motor vehicle accident claimants / NDIS participants:
You'll need a valid claim or plan and can see us at either Vic Park or Kelmscott clinic (click here to book in-clinic), otherwise we can meet you at a location suitable to you (e.g. home/local gym/rec centre). If this is your first appointment, we strongly recommend you contact us prior to booking online - we'll be able to confirm funding by your insurer. If you don't, you may be liable for any appointment costs not covered by the insurer.
